How to Submit a Photograph

If a member of our team has asked you to send a photograph, please follow the guidelines below.

  • Photographs should not be taken of areas exposing the breasts (in females) or groin/genital area (all patients) – please bear in mind that photographs will be seen by administration staff during filing
  • Photographs should be clearly focused
  • If possible, place a measuring tape or ruler next to the affected area to allow size to be determined
  • Check that the photograph gives a good representation before uploading it

Please ensure all images are in either JPEG/JPG or PDF format.

Please complete the Clinical Photography Consent Form and then send your image(s) to:

nhsh.gp55709-admin@nhs.scot

The images sent in will be stored confidentially on the practice’s secure clinical document management system and will be managed in accordance with GDPR regulations.
