New Patient Registration

Please download the Patient Registration Form at the bottom of this page along with the Preferred Pharmacy and KIS (Key Summary Information) form and you can either email it to the Practice or post it to either Surgery.  On receipt of the registration form you will receive a conformation email to confirm we have received and accepted your registration.  This process can take up to 2 weeks for you to be registered.  Once registered, you will be offered an introductory consultation with one of the doctors or nurses either by telephone or face to face.

This health check gives us the opportunity to meet you and discuss any medical details before your medical records arrive.

You will not be registered with any one particular doctor, but you can request to see a particular doctor if you so wish, it may just mean a longer wait for an appointment.

Change of Details

If you change your name, address or telephone number, please notify the surgery as soon as possible in order that we can amend your details.
